ABSTRACT

With the wide application of phacoemulsification and intraocular lens implantation, many cataract patients have effectively recovered vision. Postoperative intraocular lens opacification is an important indicator for evaluating biocompatibility, as it affects the visual quality of patients. The manifestation and risk factors of opacification vary among different materials used for intraocular lenses. However, better visual quality after surgery is not determined by a single factor. The material of the intraocular lens and the patient's response to the intraocular lens are all factors that affect the postoperative visual quality. With the continuous advancement of technology, an increasing number of new materials are being applied in the field of intraocular lenses. Fully understanding the characteristics of intraocular lens materials, selecting suitable intraocular lens for patients and reducing complications caused by materials will be beneficial to patients. The characteristics of different intraocular lens materials and the risk factors of opacification after intraocular lens implantation were discussed in this paper.

ABSTRACT Purpose: To examine the efficacy of phototherapeutic keratectomy as a treatment for variable pathologies with anterior corneal opacities and evaluate the distribution of phototherapeutic keratectomy indications over the past 10 years. Methods: The records of 334 eyes from 276 patients who underwent phototherapeutic keratectomy between March 2010 and 2020 were retrospectively reviewed. Etiologies of the patients who underwent phototherapeutic keratectomy were noted, and their changes were examined. Refractive and visual acuity results before and after the operation were recorded and analyzed according to etiology. Results: The mean age of the patients was 40.7 ± 16.2 years (range: 19-84). The mean follow-up was 25.5 ± 19.1 months (range: 3-96). Phototherapeutic keratectomy was most frequently applied for corneal stromal dystrophies (44%, 151 eyes from 111 patients), and granular dystrophy was the most common phototherapeutic keratectomy indication among corneal dystrophies. Unlike other indications, there has been an increase in the application of phototherapeutic keratectomy for persistent subepithelial opacities due to adenoviral conjunctivitis in the past 10 years. There was a significant increase in visual acuity in all groups except for the recurrent epithelial defect group (p<0.05). The greatest improvement in visual acuity was detected for stromal dystrophies in the granular dystrophy subgroup. Conclusion: Despite changing indication trends, phototherapeutic keratectomy remains an effective and reliable treatment for anterior corneal lesions.


RESUMO Objetivo: Examinar a eficácia da ceratectomia fo­toterapêutica para o tratamento de patologias variáveis que apresentarem opacidades anteriores da córnea, e avaliar a distribuição das indicações de ceratectomia fototerapêutica nos últimos 10 anos. Métodos: Foram revisados retrospectivamente os prontuários de 276 pacientes, com 334 olhos tratados com ceratectomia fototerapêutica entre março de 2010 e o ano de 2020. As etiologias dos pacientes submetidos à ceratectomia fototerapêutica foram anotadas e suas alterações foram examinadas. Os resultados refrativos e de acuidade visual antes e após a operação foram registrados e analisados de acordo com a etiologia. Resultados: A idade média dos pacientes foi de 40,7 ± 16,2 anos (faixa: 19-84). O tempo médio de acompanhamento foi de 25,5 ± 19,1 meses (faixa: 3-96). A ceratectomia fototerapêutica foi aplicada com mais frequência para distrofias estromais corneanas (44%, 151 olhos de 111 pacientes); entre as distrofias corneanas como um todo, a distrofia granular foi a indicação terapêutica mais comum desse procedimento. Ao contrário de outras indicações, nos últimos 10 anos houve um aumento na aplicação de ceratectomia fototerapêutica em casos de opacidade subepitelial persistente causada por conjuntivite adenoviral. Houve um aumento significativo na acuidade visual em todos os grupos, exceto para o grupo com defeito epitelial recorrente (p<0,05). A maior melhora na acuidade visual foi detectada em casos de distrofia estromal, no subgrupo das distrofias granulares. Conclusão: Apesar das mudanças nas tendências de indicação, a ceratectomia fototerapêutica continua sendo uma abordagem terapêutica eficaz e confiável para tratar lesões da córnea anterior.

ABSTRACT

Current COVID-19 radiological literature is dominated by CT and a detailed description of chest x-ray (CXR) appearances in relation to the disease time course is lacking. However, it poses a great challenge on the radiology department, as it is difficult to decontaminate entire CT suite(Zhao et al., 2020)11. This study is aimed to correlate clinical symptoms with the chest X-ray findings in COVID positive patients and to calculate the sensitivity and specificity of chest x ray in diagnosing COVID 19. The present study compares frequency and distribution of chest radiographic findings in COVID 19 positive symptomatic and asymptomatic patients. Patients with a RT-PCR-positive results for COVID 19 infection were 668. Of these, 437 were males (65.4%), 230 females (34.4%) and 1 transgender (0.04%) with a mean age of 44 years (range 2–87 years). Only 18 CXRs were negative for radiological thoracic involvement (6.4%) among symptomatic patients (280). Among the baseline CXR of 668 patients, 375 (56.1%) were normal and 293 (43.8%) were abnormal. Sensitivity and specificity of CXR were 93.91% and 93.3% respectively. Positive and negative predictive values were 90.97% and 95.51% respectively. We found a statistically significant relationship between CXR severity score and age, symptoms, death (P<0.001).We even found significant relationship between age and symptoms, death of COVID positive patients (P<0.001). The relationship between CXR severity and gender, age and gender were not significant (P=0.539, P=0.864 respectively)

ABSTRACT

Floaters are entopic phenomenon created by vitreous opacities. Some researchers term it as “symptomatic vitreous turbidity”, which usually refers to primary type in clinic. With the increasing prevalence and aging of myopia worldwide, vitreous floaters are becoming increasingly prevalent in clinics but receiving less attention. Floaters can impair patients' vision and quality of life due to their discomfort and disorientation. A few patients become intolerable and express a strong desire for treatment. YAG laser vitreolysis for floaters has garnered considerable attention in recent years. Although some doctors still have concerns about its efficacy and safety, a handful of studies have found some beneficial effects in recent years. The author has gained expertise in clinical practice in recent years. In this review, we talk about what causes floaters and how to classify them. We also talk about the clinical indications, how YAG laser ablation works, and whether or not it is safe.

ABSTRACT

CT screening has markedly reduced the lung cancer mortality in high-risk population and increased the detection of early-stage pulmonary neoplasms, including multiple pulmonary nodules, especially those with a ground-glass appearance on CT. Multiple primary lung cancer (MPLC) constitutes a specific subtype of lung cancer with indolent biological behaviors, which is predominantly early-stage adenocarcinoma. Although MPLC progresses slowly with rare lymphatic metastasis, existence of synchronous lesions and distributed location of these nodules still pose difficulty for the management of such patients. One single operation is usually insufficient to eradicate all neoplastic lesions, whereas repeated surgical procedures bring about another dilemma: whether clinical benefits of surgical treatment outweigh loss of pulmonary function following multiple operations. Therefore, despite the anxiety for treatment among MPLC patients, whether and how to treat the patient should be assessed meticulously. Currently there is a heated discussion upon the timing of clinical intervention, operation mode and the application of local therapy in MPLC. Based on clinical experience of our multiple disciplinary team, we have summarized and commented on the evaluation, surgical treatment, non-surgical local treatment, targeted therapy and immunotherapy of MPLC in this article to provide further insight into this field.

ABSTRACT

RESUMO A ceratopigmentação teve seu primeiro registro pelo filósofo Galeno há muitos séculos como uma estratégia utilizada para o tratamento estético de pacientes com leucomas. As córneas com leucoma são patológicas e, muitas vezes, intolerantes a lentes de contato cosméticas ou próteses oculares, sendo comum a queixa de desconforto excessivo, proporcionado pela superfície corneana irregular. Assim, a ceratopigmentação é uma alternativa para a melhora estética de pacientes com opacidades corneanas. Descrevemos o caso de um paciente do sexo masculino, 39 anos, que apresentou despigmentação precoce em caso de ceratopigmentação associado a quadro de ceratite herpética necrotizante. O paciente foi submetido ao tratamento com aciclovir 2g ao dia e doxiciclina 200mg ao dia, evoluindo com melhora do quadro clínico, apesar da má adesão medicamentosa.


ABSTRACT Keratopigmentation was first recorded many centuries ago by the philosopher Galeno, as a strategy used for the aesthetic treatment of patients with leukomas. Corneas with leucoma are pathological and often intolerant of cosmetic contact lenses or ocular prostheses, with complaints of excessive discomfort provided by the irregular corneal surface being common. Therefore, keratopigmentation is an alternative for the aesthetic improvement of patients with corneal opacities. We describe the case of a 39-year old male patient, who presented early depigmentation in a case of keratopigmentation associated with necrotizing herpetic keratitis. The patient was treated with Acyclovir 2g/day and Doxycycline 200mg/day, evolving with clinical improvement, despite poor medication adherence.

ABSTRACT

Keratopathy-associated cataract, that is, on the basis of corneal disease, and later the development of lens opacity, seriously damage visual quality. In order to avoid corneal transplantation for some patients, partial visual quality can be restored. A comprehensive and accurate evaluation of the effect of corneal opacity on visual function is of great improtance for determining cataract surgery alone. Due to the opacity of the cornea, the operation is very difficult and challenging. Therefore, it is of clinical value to develop and use new assistive technologies, including capsule staining, endoillumination, pupil dialation technology, femtosecond laser assisted technology, etc., avoiding problems such as limited visibility and decreased light flow caused by corneal opacity and facilitating cataract surgery. This article reviews progress of assistive technologies for keratopathy-associated cataract, hoping to guide clinical application.

ABSTRACT Purpose: To report the distribution of referral reasons for children from a pediatric glaucoma outpatient clinic in a tertiary eye care service. Methods: The medical records of patients aged <18 years who were referred to a pediatric glaucoma center in the city of São Paulo, Brazil, between 2012 and 2018 were retrospectively reviewed. The data collected included the referral reasons, age, hospital of origin, and who detected the ocular alteration. For defining the diagnosis, the Childhood Glaucoma Research Network classification was used. Results: Five hundred sixty-three eyes of 328 patients were included in the study. Glaucoma diagnosis was confirmed in 162 children (49%). In 83 patients (25%), the glaucoma diagnosis was ruled out, and 83 (25%) continued outpatient follow-up for suspected glaucoma. The main referral reasons were a cup-to-disc ratio >0.5 or an asymmetry ≥0.2 (24%), intraocular pressure >21 mmHg (21%), and corneal opacity (15%). In the neonatal period, the referral reasons were corneal opacity, buphthalmos, tearing, and photophobia. After the neonatal period, besides these external changes, other signs were also reasons for referral, such as cup-to-disc ratio >0.5 or asymmetry ≥0.2, intraocular pressure >21 mmHg, and myopic shift. The referrals were made by health professionals in 69% and parental concern in 30% of the cases. In 97% of the primary congenital glaucoma cases, the parents were the first to identify the change and to seek for health care. Conclusions: The referral reasons of the children to a tertiary glaucoma clinic were differed between the age groups and diagnoses. We suggest that awareness with these findings is important to avoid and postpone diagnosis, identify their impacts on prognosis, and avoid spending important resources for the management of diseases with inaccurate referrals.


RESUMO Objetivos: Relatar a distribuição dos motivos de encaminhamento de crianças para ambulatório de glaucoma infantil em um serviço oftalmológico terciário. Métodos: Dados médicos de pacientes menores que 18 anos encaminhados para ambulatório de glaucoma infantil na cidade de São Paulo, Brasil, de 2012 a 2018 foram retrospectivamente analisados. Os dados incluíram os motivos de encaminhamento, a idade, a origem e quem detectou a alteração ocular. Para definição diagnóstica, a classificação do Childhood Glaucoma Research Network foi usada. Resultados: 563 olhos de 328 pacientes foram incluídos no estudo. O diagnóstico de glaucoma foi confirmado em 162 crianças (49%). 83 (25%) pacientes tiveram diagnóstico de glaucoma descartado, e 83 (25%) continuaram em acompanhamento como glaucoma suspeito. Os principais motivos de encaminhamento foram relação escavação-disco >0,5 ou assimetria ≥0,2 (24%), pressão intraocular >21 mmHg (21%) e opacidade corneana (15%). No período neonatal, os motivos de encaminhamento foram opacidade corneana, buftalmo, lacrimejamento e fotofobia. Após o período neonatal, além desses sinais externos, outros sinais foram também motivos de encaminhamento, como escavação-disco >0,5 ou assimetria ≥0,2, pressão intraocular >21 mmHg e miopização. Os encaminhamentos ocorreram por profissionais de saúde em 69% e preocupação dos pais em 30%. Os pais foram os primeiros a identificar as alterações e procurar cuidado médico em 97% dos casos de glaucoma congênito primário. Conclusões: Os motivos de encaminhamento de crianças para um serviço de glaucoma de glaucoma terciário foram determinados e diferem em diferentes faixas etárias e grupos. Os autores reforçam a necessidade de alertar diferentes grupos para os sinais mais comuns, a fim de evitar, não só o adiamento do diagnóstico, o que impacta no prognóstico, mas também despender recursos importantes direcionados ao enfrentamento dessas doenças, com encaminhamentos imprecisos.

Abstract This study evaluated the effect of surrounded shade and specimens` thickness on the color adjustment potential (CAP) of a single-shade composite. The composite Vittra APS Unique was surrounded (dual specimens) or not (simple specimens) by a control composite (shade A1, A2, or A3). Simple specimens of the control composite were also confectioned. Opacity and whiteness index for dentistry (WID) were calculated for simple specimens. Color differences between the simple (ΔE*SIMPLE)/ dual specimens (ΔE*DUAL) and the controls were calculated. CAP was calculated based on the ratio between ΔE* SIMPLE and ΔE* DUAL. The tested composite presented lower opacity (53 to 62% vs. 80 to 93%) and higher WID (≈ 42 vs. 18 to 32) than controls. Irrespective of the specimens' thickness (1.0/ 1.5 mm), the lowest values of ΔE* SIMPLE (11.1/ 10.8) and ΔE*DUAL (7.2/ 6.1) were observed using the surrounding shade A1. The shade A3 yielded higher ΔE*SIMPLE (16.4/ 17.1) and ΔE* DUAL (11.3/ 12.3) than the A2 (ΔE*SIMPLE = 13.4/ 14.6; and ΔE* DUAL = 9.7/ 10.3). The specimen`s thickness significantly affected the CAP (0.35 and 0.44 for 1.0 and 1.5 mm, respectively) only for shade A1, which had the highest CAP values. The shade A3 resulted in higher CAP values (0.31) than A2 (0.27) when 1.0-mm thick specimens were used, but similar values were observed for 1.5 thick specimens (≈ 0.29). In conclusion, both surrounding shade and specimen thickness can affect the CAP of a single-shade resin composite.


Resumo Este estudo avaliou o efeito da cor circundante e da espessura da amostra no potencial de ajuste de cor (PAC) de uma resina monocromática. O compósito Vittra APS Unique foi circundado (amostras duplas) ou não (amostras simples) com um compósito controle (cor A1, A2 ou A3). Amostras simples do compósito controle também foram confeccionadas. A opacidade e o índice de brancura para odontologia (IBO) foram calculados para as amostras simples. As diferenças de cor entre amostras simples (ΔE*SIMPLE) / duplas (ΔE*DUAL) e os controles foram calculados. PAC foi calculado baseado na razão entre ΔE* SIMPLE e ΔE* DUAL. O compósito testado apresentou menor opacidade (53 a 62% vs. 80 a 93%) e maior IBO (≈ 42 vs. 18 a 32) que os controles. Independente da espessura da amostra (1,0/ 1,5 mm), os valores mais baixos de ΔE* SIMPLE (11,1/ 10,8) e ΔE*DUAL (7,2/ 6,1) foram observadas quando a resina circundante era A1. A cor A3 resultou em maior ΔE*SIMPLE (16,4/ 17,1) e ΔE* DUAL (11,3/ 12,3) do que A2 (ΔE*SIMPLE = 13,4/ 14,6; e ΔE* DUAL = 9,7/ 10,3). A espessura das amostras afetou significativamente o PAC (0,35 e 0,44 para 1,0 e 1,5 mm, respectivamente) apenas para a cor A1, que teve os maiores valores de PAC. A cor A3 resultou nos maiores valores de PAC (0,31) que A2 (0,27) quando amostras de 1,0 mm de espessura foram usadas, mas valores similares foram observados para amostras de 1,5 mm (≈ 0,29). Como conclusão, tanto a cor circundante como a espessura das amostras podem afetar o PAC de um compósito resinoso monocromático.

Si en el período temprano de la vida ocurre alguna condición que no permita una adecuada estimulación visual se produce la ambliopía por deprivación. Las afecciones corneales que se diagnostican en los niños pueden tener un origen congénito o adquirido, este último traumático o no. El conocimiento del diagnóstico de las afecciones que ocasiona opacidad corneal en niños es determinante para poder obtener todos los elementos necesarios que ayuden a identificar un diagnóstico lo más temprano y preciso posible y proveer una adecuada guía para escoger estrategias de tratamiento, por parte del equipo multidisciplinario involucrado. El objetivo fue abordar de manera actualizada las características en el diagnóstico y tratamiento de las opacidades corneales en edades pediátricas. Se realizó una búsqueda automatizada sobre el tema teniendo en cuenta las publicaciones de los últimos cinco años. Se utilizó la plataforma Infomed, específicamente la Biblioteca Virtual de Salud(AU)


If any medical condition that does not allow adequate visual stimulation manifests early in life, deprivation amblyopia follows. Corneal conditions diagnosed in children may have a congenital or acquired origin, the latter being traumatic or not. Knowledge of the diagnosis of conditions that cause corneal opacity in children is crucial to obtain all the necessary elements to help identify the earliest and most accurate diagnosis possible and to provide an adequate guide to choose treatment strategies by the multidisciplinary team involved. The objective was to address in an updated way the characteristics in the diagnosis and treatment of corneal opacities in pediatric ages. An automated search on the subject was carried out taking into account the publications of the last five years. The Infomed platform was used, specifically the Virtual Health Library(AU)

ABSTRACT

Background: Cataract and corneal blindness continue to be leading causes of reversible blindness in India. These can co-exist in a multitude of pathologies such as trauma, healed keratitis (old herpetic scar), chronic degenerative changes such as labrador keratopathy, bullous keratopathy, corneal dystrophies etc. Phacoemulsification in such eyes is rewarding to the patient in terms of minimal intervention, less risk of complications owing to reduced open sky time (as in case of combined keratoplasty), and better predictable visual outcomes. Approach to such eyes with poor visualisation is highly challenging. Purpose: We illustrate a modified surgical technique of chandelier illumination through pars plana for cataract surgery in eyes with corneal opacity of varying grades. Synopsis: Five patients with dense cataract and small pupils, associated with corneal opacity (leucomatous and macular grade) are described. Closed chamber phacoemulsification with intraocular lens with or without pupil expanders was performed assisted by 23 or 25 gauge pars plana chandelier illumination introduced in the vitreous cavity through a sclerotomy wound made prior to phacoemulsification in the inferotemporal quadrant. Highlights: Chandelier illumination aids in reducing the light scatter that occurs due to corneal opacity. Ease of visualisation of lens structures and of performing cataract surgery was noticed. One case was combined with penetrating keratoplasty with reduced open sky time. This assisted technique has advantages such as enhancing visualisation intraoperatively and allowing working in closed chamber. Its self-retaining nature aids bimanual manipulation. No complications were encountered. The video highlights the utility, advantages and practicality of chandelier retroillumination in patients with corneal opacities of varying degree undergoing phacoemulsification.

Purpose: To analyze the morphological outcomes of the posterior corneal opacity or 搒emilunar sign� in noninfectious anterior scleritis using multimodal imaging. Methods: This was a prospective observational case series. Patients with anterior scleritis from January 2018 to January 2019 were included. Clinical and demographic data were collected. Posterior cornea was visualized using the digital slit lamp photography (Elite, mega digital vision), spectral domain optical coherence tomography (MS39), and specular count analyzer (EM?3000). 揝emilunar sign� was defined by the (1) presence of posterior corneal opacity, (2) concave semilunar pattern, (3) absence of blood vessels, and (4) normal anterior cornea. Incidence, clinical characteristics and significance, correlation with Mantoux sensitivity, and role of multimodal valuation were assessed. Results: Overall 76 eyes of 72 patients were recruited with anterior scleritis. Fifteen eyes of 11 patients (15.3%) presented with semilunar sign. The scleritis was both nonnecrotizing (n = 8) and necrotizing (n = 7). The semilunar configuration appeared as isolated (n = 9) and continuous lesion (n = 6). The extent was directly related to the scleral disease extent (P = 0.002). The mean thickness measured 212.5 � 129.3 ?m. The mean central endothelial cell density (ECD) was 2540.8 � 351.7 cells/mm2, which was significantly higher than the involved peripheral cornea (P = 0.05). The mean surface area of the semilunar sign was 7.7 � 5.2 mm2. There was no significant correlation between the opacity thickness and the best?corrected visual acuity (P = 0.895, r = ?0.39), ECD (P = 0.52, r = ?0.188), and Mantoux (P = 0.696, r =? 0.142). Conclusion: Corneal semilunar sign of scleritis affected the peripheral cornea and caused no functional abnormality in early presentation. Multimodal analysis can aid in clinical assessment and severity.

RESUMO Objetivo: Relatar, por meio de uma série de casos, a percepção de pacientes com opacidade corneana sobre a eficácia da tatuagem na melhoria estética de seus olhos, utilizando a combinação de duas técnicas. Métodos: Oito pacientes responderam a um inquérito sobre sua satisfação estética com o procedimento, o desconforto pós-operatório e o impacto social observado após a cirurgia. Resultados: Todos os pacientes consideraram-se muito satisfeitos com os resultados. Em relação ao grau de desconforto no pós-operatório, 75% disseram ter tido pouco desconforto, e 25% relataram desconforto moderado. Todos os pacientes relataram melhora significativa no bem-estar social e pessoal. Da mesma forma, todos os pacientes disseram que repetiriam o procedimento. Conclusão: A tatuagem corneana surge como um método alternativo às lentes de contato e às próteses oculares em pacientes cegos com leucomas, trazendo resultados estéticos satisfatórios, duradouros e que podem promover impactos sociais na vida desses pacientes.


ABSTRACT Objective: To report, through a case series, the perception of patients about the effectiveness of corneal tattooing in the cosmetic improvement of their eyes with leukomas, using a combination of two techniques. Methods: Eight patients answered a survey regarding their cosmetic appearance satisfaction regarding the procedure, postoperative discomfort, and social impact observed after surgery. Results: All patients considered themselves very satisfied with the cosmetic results. Regarding the degree of postoperative discomfort, 75% said they had little discomfort, while 25% reported moderate discomfort. All patients reported significant improvement in social and personal well-being. Likewise, all patients said they would repeat the procedure. Conclusion: Corneal tattooing appears as an alternative method to contact lenses and ocular prostheses in impaired eyes with leukomas, bringing satisfactory and long-lasting cosmetic improvement that can promote social impacts for these patients.

ABSTRACT

Objective: To determine the effect of the restorative technique, material's opacity and the selected light sources on the curing potential (CP) in deep cavities. Material and Methods: The radiant exitance (mW/cm2) of two light curing units (Bluephase G2 and Radii-Cal) was determined at 0 and 8 mm distance from a power meter sensor (Ophir). Two bulk-fill composites of regular consistency (Opus, FGM; and Filtek One, 3M) and a conventional one (Sirius-Z, DFL) were considered and the level of their opacity were determined by a sphere-based spectrophotometer (SP60, X-Rite). The degree of C=C conversion (DC) was determined by spectroscopy (FTIR-ATR) at 0.05 and 8 mm depths and the CP considered the ratio between them. The "incremental technique" considered 4 increments of 2mm thickness each, whereas the "bulk-fill technique considered 2 increments of 4mm-thickness. Data were submitted to analysis of variance and Tukey's test (95%). Pearson's correlation tests were performed to calculate the possible relation between curing potential and materials' opacity. Results: With the incremental technique it was possible to achieve high CP regardless of the light source. The CP of bulk-fill composites was dependent on the light source, whereas Radii-Cal compromised the DC at 8 mm for both materials. The correlation between opacity and CP was dependent on the light source (r = 0.891707246 for Radii-Cal; r = 0.515703768 for Bluephase G2). Conclusion: The bulk-fill technique was dependent on the light source while the incremental was not. The influence of materials' opacity was dependent on the light curing unit.(AU)


Objetivo: Determinar o efeito da técnica restauradora, a opacidade do material e as fontes de luz selecionadas sobre o potencial de cura (PC) em cavidades profundas. Materiais e métodos: A saída radiante (mW/cm2) de duas unidades de cura por luz (Bluephase G2 e Radii-Cal) foi determinada a 0 e 8 mm de distância de um sensor de potência (Ophir). Dois compósistos bulkfill de consistência regular (Opus, FGM; e Filtek One, 3M) e um convencional (Sirius-Z, DFL) foram considerados e o nível de sua opacidade foi determinado por um espectrofotômetro (SP60, X-Rite). O grau de conversão C=C (DC) foi determinado por espectroscopia (FTIR-ATR) a 0,05 e 8 mm de profundidade e o PC considerou a relação entre eles. A técnica incremental considerou 4 incrementos de 2 mm de espessura cada, enquanto que a técnica bulkfill considerou 2 incrementos de 4 mm de espessura. Os dados foram submetidos à análise de variância e ao teste de Tukey (95%). Os testes de correlação de Pearson foram realizados para calcular a possível relação entre o potencial de cura e a opacidade dos materiais. Resultados: Com a técnica incremental foi possível obter um PC elevado, independentemente da fonte de luz. O PC de compósitos bulkfill foi dependente da fonte de luz, enquanto que Radii-Cal comprometeu o DC em 8 mm para ambos os materiais. A correlação entre opacidade e PC foi dependente da fonte de luz (r = 0,891707246 para Radii-Cal; r = 0,515703768 para Bluephase G2). Conclusão: O potencial de polimerização para a técnica bulkfill foi dependente da fonte de luz, enquanto para a técnica incremental não. A influência da opacidade dos materiais sobre a capacidade de polimerização foi dependente da unidade de fotoativação. (AU)

ABSTRACT

Objective: Resin-modified glass ionomer (RMGI) cements are among the commonly used restorative materials in low stress-bearing areas and also for temporary restorations. The competition between acid-base reactions and light polymerization reactions in delayed curing of RMGIs can affect their physical and mechanical properties, as well as their degree of conversion. Since solubility, color stability, and opacity are among the main physical properties affecting the durability and clinical service of RMGI restorations, this study aimed to assess the effect of delayed curing on solubility, color stability, and opacity of Fuji II LC RMGI. Material and Methods: This in vitro, experimental study evaluated 80 Fuji II LC RMGI specimens (10 specimens per each in 4 groups) in terms of solubility, color stability, and opacity at 6 months later. Specimens were cured immediately or were cured with 1, 5 and 10 min delay. Results: Maximum solubility and minimum change in opacity and color stability at 6 months were noted in the group with delayed curing by 10 min. A significant difference was noted in the solubility of specimens cured after 10 min and 1 min. Significant differences were also noted in the opacity and color stability of specimens cured after 10 min and all other groups (P < 0.05). Conclusion: Delayed curing by 1 min decreased the solubility of RMGI specimens compared with immediate curing or curing after 5 min. Although this difference did not reach statistical significance. Color stability and changes in opacity are mainly influenced by the acid-base reactions rather than polymerization reactions.(AU)


Objetivo: Cimentos de ionômero de vidro modificado por resina (CIVMR) estão entre os materiais restauradores mais comumente utilizados em áreas de baixa tensão e também para restaurações temporárias. A competição entre reações ácido-base e reações provenientes da fotopolimerização tardia dos CIVRMs podem afetar suas propriedades físicas e mecânicas, bem como seu grau de conversão. Uma vez que a solubilidade, estabilidade de cor e opacidade estão entre as principais propriedades físicas que afetam a durabilidade e o tempo de serviço clinico de restaurações de CIVMR, este estudo teve como objetivo avaliar o efeito da fotopolimerizaçao tardia na solubilidade, estabilidade de cor e opacidade do CIVMR Fuji II LC. Material e Métodos: Este estudo experimental in vitro avaliou 80 espécimes de CIVMR Fuji II LC (4 grupos com 10 espécimes cada) em termos de solubilidade, estabilidade de cor e opacidade apos 6 meses. As amostras foram fotopolimerizadas imediatamente ou com 1, 5 e 10 min de atraso. Resultados: Máxima solubilidade e mínima alteração na opacidade e estabilidade da cor em 6 meses foram observadas no grupo com fotopolimerização tardia em 10 min. Uma diferença significativa foi observada na solubilidade das amostras fotopolimerizadas após 10 min e 1 min. Diferenças significativas também foram observadas na opacidade e estabilidade de cor das amostras fotopolimerizadas após 10 min e em todos os outros grupos (P <0,05). Conclusão: A fotopolimerizaçao tardia em 1 min diminuiu a solubilidade das amostras CIVMR em comparação com a fotopolimerizaçao imediata ou após 5 min. Embora essa diferença não tenha alcançado significância estatística. A estabilidade da cor e as mudanças na opacidade são influenciadas principalmente por reações ácido-base, em vez de reações causadas pela polimerização(AU)

ABSTRACT

@#Along with the popularity of low-dose computed tomography lung cancer screening, an increasing number of lung ground-glass opacity (GGO) lesions are detected. The pathology of GGO could be benign, but persistent GGO indicates early-stage lung cancer. Distinct from traditional lung cancer, GGO-featured lung cancer is more common in the young, nonsmokers and females. GGO-featured lung cancer represents an indolent type of malignancy with a long time to intervene. However, there is still no consensus on the screening, pathology, surgical procedure, and postoperative surveillance of GGO-featured lung cancer. Therefore, we proposed a personalized treatment strategy for GGO-featured lung cancer. The screening for GGO-featured lung cancer should be conducted at young age and low frequency. Adenocarcinoma in situ, minimally invasive adenocarcinoma, lepidic, and non-lepidic growth patterns could present as GGO. The following issues should be taken into consideration while determining the treatment of GGO-featured lung cancer: avoiding treating benign disease as malignancies, avoiding treating early-stage disease as advanced-stage disease, avoiding treating indolent malignancy as aggressive malignancy, and choosing appropriate timing to receive surgery without affecting life tracks and career developments. Bronchoscope and bone scan are not necessary for preoperative examinations of GGO-featured lung adenocarcinoma. For selected patients, sublobar resection without mediastinal lymph node dissection might be sufficient. Intraoperative frozen section is an effective method to guide resection strategy. Given the excellent survival of GGO-featured lung cancer, a less intensive postoperative surveillance strategy may be sufficient.

ABSTRACT

The lens of the eye has been recognized as one of the most radiosensitive tissues. It has been known that a radiation dose of 0.5 Gy or higher can induce radiation cataract. Studies have also demonstrated that long-term exposure to low-dose radiation can increase the risk of lens opacity. However, the specific biologic mechanism of lens opacity induced by low-dose ionizing radiation is still unclear. Many mechanism chains may induce opacity independently or jointly, such as genomic damage in lens epithelial cells, oxidative stress, defects in intercellular communication, and inflammatory reaction, and genetic and epigenetic factors may also play a role. This paper briefly reviews the mechanisms of low-dose ionizing radiation inducing lens opacity as well as the role of genetic susceptibility in its development and progression, in order to provide a scientific reference for the prevention and control of this disease.

Small leucine-rich proteoglycans (SLRPs) are necessary structural ingredients of the cornea, which are vital for the establishment and maintenance of corneal transparency.SLRPs are mainly located in the corneal stroma and can be divided into class Ⅰ, class Ⅱ, and class Ⅲ.The compensatory and cooperative interactions among SLRPs regulate the formation and assembly of stromal collagen fibrils, thereby maintaining the highly ordered arrangement of collagen fibrils, and establishing corneal transparency.Decorin and lumican are the main functional components of class Ⅰ and class Ⅱ SLRPs, respectively, and changes in their expression or abnormities in the structure of their core proteins affect the natural content and arrangement of other stromal extracellular matrix components, ultimately resulting in abnormal fibril formation, assembly, and arrangement, causing corneal opacity.SLRPs can regulate corneal wound healing and stromal matrix remodeling via binding to fibrotic molecules and their receptors, which provides bases for corneal diseases therapy and study of molecular mechanisms of corneal transparency.The bioactivities and the role of SLRPs in corneal transparency were reviewed in this article.

ABSTRACT

Objective To establish bovine corneal opacity and permeability (BCOP) test, and determine its predictive ability for the eye irritation evaluation of cosmetics. Methods A total of ten reference chemicals were selected to establish the BCOP test. Then eye irritation of 16 routinely collected cosmetics in our laboratory was predicted. In vitro scores were calculated by the change in the opacity and sodium fluorescein permeability after exposure to the testing cosmetics, and subsequently compared with the historical data by Draize test. Results Reference chemicals with known irritation classification were correctly classified by the BCOP test, which was consistent with the classification of UN globally harmonized system of classification and labeling of chemicals. Moreover, the specificity of the BCOP test for the classification of non-irritating cosmetics samples was 80.0% (8/10), and the sensitivity for weak to mild irritating cosmetics samples was 83.3% (5/6). The BCOP test demonstrated an overall classification consistency of 81.3% (13/16) with in vivo test. Conclusion BCOP test may be independently used to identify chemicals with potential eye irritation and serious eye damage, suggesting it is significant for in vitro integrated test strategy for predicting eye irritation due to cosmetics.

ABSTRACT

OBJECTIVE@#To compare the effects of different imaging methods on radio-opacity. Digital imaging and traditional film imaging were used to determine the radio-opacity of dental composite resin materials.@*METHODS@#Fourteen dental composite resins commonly used in clinic were prepared for disc samples with thickness of 1 mm and diameter of 15 mm respectively. The aluminum step wedge was used as the reference and the disc samples were irradiated with X-rays by the traditional film imaging, phosphor imaging plate and charge-coupled device(CCD) intra-oral X-ray sensor respectively. Exposure was set at 65 kV and 7 mA, with 300 mm focus to film distance and the exposure time was 0.25 s. After developing and fixing the film, the optical density of the image of the sample and that of each step of the aluminum step wedge were measured using the densitometer. The digital image file was exported to grey scale analysis software of Adobe Photoshop® to measure the average grey value in the sample image and aluminum step wedge image. The curves were drawn corresponding to the optical density/gray value of each step of the aluminum step wedge and its thickness, and the equivalent thickness of the aluminum plate was calculated according to the optical density/gray value of the actual measured thickness of the sample, so as to evaluate the radio-opacity of dental composite resin material.@*RESULTS@#For the same sample, the aluminum equivalent measured by traditional film imaging and two other digital imagings had significant differences among the 14 composite resins (P < 0.05), and the aluminum equivalent measured by the two digital imagings were greater than that of the traditional film. Moreover the aluminum equivalent measured by the two digital imagings was also different, and the aluminum equivalent measured by the CCD intra-oral X-ray sensor in most composite resins was larger than that measured by phosphor imaging plate. The same sample was photographed with the same imaging method, and the aluminum equivalent was different after three exposures. The standard deviation of aluminum equivalent measured by phosphor imaging plate was the largest, while that measured by CCD intra-oral X-ray sensor was the smallest. Among the 14 dental composite resin materials, the aluminum equivalent of Tetric N-Ceram and Te-Econom Plus measured by traditional film imaging and phosphor imaging plate was significantly higher than other composite resins.@*CONCLUSION@#Based on clinical significance, three kinds of sensors designed to measure the radio-opacity of dental composite resin, the value of aluminum equivalent will change according to different types of sensors. The aluminum equivalent measured by CCD intra-oral X-ray sensor was higher than that measured by phosphor imaging plate, and the aluminum equivalent measured by phosphor imaging plate was higher than that measured by traditional film imaging. Moreover, even though the same sensor was used, the aluminum equivalent measured by multiple exposures was different. The standard deviation of the aluminum equivalent measured by phosphor imaging plate was greater than that measured by traditional film imaging and CCD intra-oral X-ray sensor. The three sensors in this study could be used for evaluating the radio-opacity of dental composite resin materials. But no matter what kind of sensor was used to measure the radio-opacity of dental composite resin, it complied to ISO 4049: 2019 standard, the materials were suitable for clinical use.
